Voiceless nasals in the Ikema dialect of Miyako Ryukyuan

Voiceless nasal consonants are typologically rare in the world’s languages. The present study investigates the acoustic realization of reported voiceless nasals in the Miyako Ryukyuan dialect Ikema. Voiceless nasals in Ikema occur word-initially and word-medially as part of a geminate or consonant cluster, and are phonemically distinct from modal voiced nasals. Initial observation of collected recordings revealed many instances of the voiceless phoneme with voicing throughout, leading to a re-evaluation of previous claims about its phonetic implementation. We hypothesized that word-medial and phrase-medial voiceless nasals surface as breathy voiced nasals. We analyzed the acoustic characteristics of nasal components of target words, focusing on duration, phonation state, and cepstral peak prominence (CPP), to determine whether reported voiceless nasal phonetic components with voicing are acoustically distinct from modal voiced nasal consonants. We find that voiceless nasals are produced with a voiceless component followed by a modal voiced component. Voiceless components and breathy components are found to be significantly shorter than modal components. We also find a significant difference between modal nasal, breathy nasal and voiceless nasal components’ CPP values. The results confirm the observation that Ikema voiceless nasals are phonemically distinct from modal nasal consonants, and likely allophonically vary with breathy voiced nasals word-medially and phrase-medially. These findings align with the hypothesis that voiceless nasals require some voicing to be audible for perception, and are consistent with cross-linguistic findings, contributing to the typological understanding of the acoustics of voiceless nasals.
Exploring the relationship between pain and self-harm thoughts and behaviours in young people using network analysis

Background
Self-harm thoughts and behaviours (SHTBs) are a serious public health concern in young people. Emerging research suggests that pain may be an important correlate of SHTBs in young people. However, it remains unclear whether this association is driven by the shared association with other correlates of SHTBs. This study used network analysis to delineate the relationship between SHTBs, pain and other correlates of SHTBs in a population-based sample of young people.
MethodsWe performed secondary analyses, using data from 7977 young people aged 5–16 years who participated in the British Child and Adolescent Mental Health Survey in 2004. We used χ2 tests and network analysis to examine the complex interplay between SHTBs, pain and other correlates of SHTBs, including psychiatric disorders, childhood trauma, stressful life events, parental distress, family dysfunction, peer problems and inhibitory control deficits.
ResultsPain was associated with a doubled risk of SHTBs, and likewise, SHTBs were associated with a doubled risk of pain. Furthermore, network analysis showed that although pain was significantly associated with all measured correlates of SHTBs, except family dysfunction, pain was most strongly associated with SHTBs, after accounting for these measured correlates.
ConclusionsTo the best of our knowledge, this is the first study to utilise network analysis to provide novel insights into the complex relationship between SHTBs, pain and other known correlates of SHTBs in young people. Results suggest that pain is an independent correlate of SHTBs. Future research should aim to identify underlying mechanisms.

Background
Schizophrenia affects 1% of the population. Clozapine is the only medication licensed for treatment-resistant schizophrenia and is intensively monitored to prevent harm from neutropenia. Clozapine is also associated with increased risk of pneumonia although the mechanism is poorly understood.
AimsTo investigate the potential association between clozapine and antibody deficiency.
MethodsPatients taking clozapine and patients who were clozapine-naive and receiving alternative antipsychotics were recruited and completed a lifestyle, medication and infection-burden questionnaire. Serum total immunoglobulins (immunoglobulin (Ig)G, IgA, IgM) and specific IgG antibodies to haemophilus influenzae type B, tetanus and IgG, IgA and IgM to pneumococcus were measured.
ResultsImmunoglobulins were all significantly reduced in the clozapine-treated group (n = 123) compared with the clozapine-naive group (n = 111). Odds ratios (ORs) for a reduction in clozapine:control immunoglobulin values below the fifth percentile were IgG, OR = 6.00 (95% CI 1.31–27.44); IgA, OR = 16.75 (95% CI 2.18–128.60); and IgM, OR = 3.26 (95% CI 1.75–6.08). These findings remained significant despite exclusion of other potential causes of hypogammaglobulinaemia. In addition, duration on clozapine was associated with decline in IgG. A higher proportion of the clozapine-treated group reported taking more than five courses of antibiotics in the preceding year (5.3% (n = 5) versus 1% (n = 1).
ConclusionsClozapine use was associated with significantly reduced immunoglobulin levels and an increased proportion of patients using more than five antibiotic courses in a year. Antibody testing is not included in existing clozapine monitoring programmes but may represent a mechanistic explanation and modifiable risk factor for the increased rates of pneumonia and sepsis-related mortality previously reported in this vulnerable cohort.

Auditory function at 14 years of age of very-low-birthweight children

The aim of the study was to determine audiological function at 14 years of age of very-low-birthweight (VLBW [les ]1500g) children compared with a cohort of normal birthweight (NBW >2499g) children. Participants were consecutive surviving preterm children of birthweight <1000g born between 1977 and 1982 (n=86) and of birthweight 1000 to 1500g born between 1980 and 1982 (n=124) and randomly selected NBW children born between 1981 and 1982 (n=60). Audiometric tests included pure tone audiometry, tympanometry, stapedius muscle reflexes, and measures of central auditory processing. Psychometric tests included measures of IQ, academic achievement, and behaviour. There were no significant differences in rates of hearing impairment, abnormal tympanograms, figure–ground problems, or digit recall between VLBW children and NBW control children. VLBW children had higher rates of some central auditory processing problems, which in turn were associated with poorer intellectual, academic, and behavioural progress.
